Online Portal You can also fill out the paperwork on our online portal. There is a short delay in us getting your paperwork, but you do not need a printer for this option.
- Online Portal: Register and fill out your paperwork online at Western Surgical Group (our parent company) website.
- Patient Health History: Once you have logged in and registered, go to “Patient Health History” at the bottom of the page and complete.
- In order to expedite your process, please email or call and let us know you when you have submitted your paperwork. You can also inform us through the online portal by “Asking a Non-Urgent Clinical Question.”
- When asked for your Preferred Provider, if you are unsure at this time, pick option “Physician not on list” and we will discuss with you later based on availability.
- When asked about the location in the consent form, pick “WBI-645 Arlington Ave.”
Consultation: If you haven’t heard from us, you can go to the online portal and inquire by “Ask a Non-Urgent Clinical Question” or call the office at 775-326-9152 option # 3.
